**Description**



Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a member of the Case Management Department, works collaboratively with the interdisciplinary team to effect positive patient and family outcomes. Serves as resource for social workers and others within the team.



**Required qualifications:**



+ Master's Degree in Social Work from an accredited graduate school of social work.

+ Texas Masters Social Worker License - Texas State Board of Social Worker Examiners upon hire, Or

+ Texas Masters Social Worker Temporary License - Texas State Board of Social Worker Examiners upon hire



**Preferred qualifications:**



+ 1 year Minimum of one year experience delivering social work services in a healthcare setting.

+ Post degree experience in a hospital or other health care setting.
